Mailing Process: Send an acceptable copy of the State Commission certificate, the original $15,000 surety bond (properly signed by the applicant), two copies of the Oath form (originally signed and executed by the local administering notary), a cover letter with your contact info, a check or money order, and a self-addressed stamped envelope.
