Mail Filing Option

Send a copy of the Secretary of State commission certificate, two completed copies of the Oath of Office (fully executed by the local administering notary), the original signed $15,000 surety bond, a cover letter with the student’s daytime phone number, a check or money order for the exact amount ($25.00 baseline), and a self-addressed stamped envelope.
